Churn Rate - urgent help needed

Hello, I have a data model with the following columns and I am trying to find both the MONTHLY churn rate AND ANNUAL churn  rate and create 2 text boxes which just states the percentage. Can someone please advise what measures I have to write?  (I also have a calendar table which is connected through non-active relationships to both date columns.)

My expected output is a single % number (measure) for each (monthly and annual % rates) that I can put on a text card

I am a super beginner here - I am really looking for someone to help me write out dax formulas for my specific data needs. There is more complexity here as I have multiple date columns I am referencing
