For some reports I notice that there is an unusual count of views for 1 useraccount in the usage metrics report.
Does view counts include the refreshes as well, ie the owner of the dataset gets an view count each time the dataset is refreshed,
or ...?

Based on your description, I looked up the documentation. First, the view count in the usage metrics does not include dataset refreshes. The view count is only incremented when the user actively opens the report. The refresh operation only affects the dataset behind the report, but is not counted as a view.
Exception counts using metrics may be due to the fact that using metrics may sometimes overcount activity counts for cases where a client refreshes without sending a request back to the Power BI service. For example, switching report pages does not send a report load request to the server because the page definition is already in the browser.
It is also possible that using metrics may sometimes undercount activity because of inconsistent network connections, ad blockers, or other issues that may interrupt the sending of events from the client.
