Want to fill the data on condition basis in matrix

Dear's,

I am having a data as shown below and, I created an index to identify whether the code is null or not. Coz, I will always get type x code = blank.

The output which i am getting as per the data is correct (Ref: Below screenshot)

Whereas, the requirement is: I need to fill the X type value for each code based on "st" and "misdate" (Ref: Below screenshot). I tried with pivoting, but i couldn't succeed, also i tried to use switch command to fill the data but i couldn't. 

Looking expert suggestion. Thanks in advance.
