Filter table based on measured column

I have a Supplier table which contains data such as address line 1, address line 2, address line 3 ect.. each on of these columns has a new measured column against which is populated as either a 0 or 1 depending on if the criteria of the address passes or fails. 

 

I have then created a slicer and using the measured columns i want to be able to filter the table on what is selected in the slicer. However, this slicer is now putting the measured columns within a heirachal grouping which is very confusing. How can this be resolved just to the slicer would output the columns so one can be selected, and the table would be filtered by it. Output as ;

 

- Address 1 Pass

- Address 1 Fail

- Address 2 Pass

- Address 2 Fail

- Address 3 Pass

- Address 3 Fail

measured column

That's not a thing.  It's either a calculated column or a measure.  

 

Please provide sanitized sample data that fully covers your issue. I cannot help you without usable sample data.
Please paste the data into a table in your post or use one of the file services like OneDrive or Google Drive. I cannot use screenshots of your source data.
Please show the expected outcome based on the sample data you provided. Screenshots of the expected outcome are ok.
