I might just have a hard time finding the documentation, but what exactly causes the URL to change on a published report I am using? Or is it easier to answer the inverse, what updates can I do that don't change the URL of the published report?

The short answer is that once a report is given a URL, that is the URL for that report going forward. It doesn't change. Now, if you delete/remove the report and publish it as a new report, that is a new URL. Regular re-publish does not change the URL.

Does Publishing the same report from a different machine (they use the same PBI file) create a new URL

I have done this numerous times and have even had other folks modify and re-publish a report without the URL changing. As long as you are overwriting the existing report the URL should never change.
