Yes, Power BI Report Server is available through two different licenses: Power BI Premium and SQL Server Enterprise Edition with Software Assurance. With a Power BI Premium license, you can create a hybrid deployment mixing cloud and on-premises.
For Power BI Premium, Power BI Report Server is only included with P SKUs. It is not included with EM SKUs.

No, you should purchase premium plan based on your requirement because power bi report server is included in premium plan so it's not free.
No, publish to report server or service is free, however, if you want to share reports with others or create app in power bi serbice, you should purchase pro license.

You talking about premium though.
I am talking about Report Server on Premise. I want to confirm that this is free if you have SQL server Enterprise version. Client does not want to purchase premium. The plan is to use Report Server on Premise (free under SQL Enterprise) and then purchase Pro licenses.
