Not sure if this is the right forum, but I am wondering what are the requirements for the Power BI Gateway? Does it need it's own server? What are the best practices for setup and resources required? Is it needed only for on-prem data connections, or do cloud connections go through the gateway as well?

No, you can just install it on the server that can connect to your on-premise data source. Based on my experience, you'd better install it on the same server that hold the data source.

No, it's not necessary. Only if you contain on-premise data source and cloud data source in one dataset, and you have query to combine these two data source, the cloud data source need to use gateway as well. You can configure it in settings of gateway in power bi service.

Here is the recommendation, and as long as you have a server with enough resources for CPU, Memory and bandwidth you should be good to go.
I would recommend a seperate server as close to the data sources as possible.
