Do apps need to be manually updated every time a report changes? My clients are not seeing any changes made on the report if I don't manually click on the updated App. This doesn't make any sense. There is not a way to keep it automatic??

That is by design.
The reason is that a change of a report can be uploaded to the App Workspace. This will then allow others to verify and ensure that the changes are good. And then the final step is to then update the app to reflect the new changes.
Whilst it is an extra step after the report has been published it is a quick update all and it is good to go!

Sorry there is no automatic way to update the app so far, you have to manually update the app when needed to reflect your changes.
As a workaround:
You can create a new workspace containing all the content in this app, and grant users the role of Viewer in the workspace so they can access your workspace. (Just like read-only access). In this way, they can see all the items you have updated in the workspace, not just the items published through the app.
Also you can submit an idea for it in Power BI Idea forum to help improve this feature.
