How can I transpose 2 dimensions data to rows

Hi everyone, I'm sorry for my bad English. I've tried to find the solution for this but didn't work so I post this message for your help.

I have this data:

Name  

   a           11/1     12/1     13/1

                 a1         a2        a3

   b           1/2        2/2      3/2

                 b1         b2        b3

   c           2/1        3/1      4/1

                 c1         c2        c3

What I'm looking for

a   11/1  a1 

a   12/1  a2

a   13/1  a3

b   1/2   b1

b   2/2   b2

b   3/2   b3

c   2/1   c1

c   3/1   c2

c   4/1   c3
