We deleted one of the reports in a workspace using powerbi portal.
However, using the same user account with the api, we still see the report.

As a test we deleted all the reports and datasets in the workspace. The report still shows up through the API.
What would cause a report that has been deleted in the portal to still exist using the api? (again, using the same user account)
Based on my test, as long as we delete the report in Power BI service manually, the API Reports - Get Report In Group will return "PowerBIEntityNotFound" as below:
Please test again on your side to see if the same issue occurs.

We deleted the report manually as well. Thinking it would work itself out, but even now in the UI we have 5 reports, but using the API (same user account) we have 6 reports.
I could try deleting it using API
It looks a little strange. I would suggest you create a support ticket to let engineers look into the issue on your side.
