We use the Template App process daily with many customers and it began failing on 10/18/2020.
When anyone in our Power BI tenant attempts to create a new, or update an existing, Template App it hangs on the "Authentication" screen with a "Loading..." message.
We can see that both the "Authentication" screen and the "Parameters" screen were updated recently by Microsoft to have new fields. Is it possible that this Microsoft update caused an issue?

Please note: the experience you're describing is indeed part of the new improvements to app consumption & authoring.
TL;DR: A basic rules of thumb: if the workspace works as expected and refreshes - the application should work.
Longer explanation:
These improvements are intended to ensure a seamless installation experience for users. This has added some validations on the app author as a pre-requisite.
The error you're experiencing is due to the fact that the dataset as it is defined in the workspace has not been configured to successfully connect to any datasource (nor refresh in that workspace).
You can workaround this issue by making sure the dataset indeed connects and refreshes in that workspace (by going to dataset settings and configuring parameters & credentials).
We are working on improving this new experience for these types of errors while allowing for easier authoring of new applications.
please note: setting default values when creating the application, does not affect the values as they are set in the workspace.

I am having the same issue while creating an app.
I tried to publish and create the app without the python script, it worked well.
Thanks,
