Unpivot and Add an Additional Category Field

Hi guys,

This is my 1st post on here so hopefully the 1st of many 😀

I've been trying without success to shape the table data below in to something workable.

Each column after the 1st 3 has a monthly value, however contained within each month is a category of "Blended", "Fixed Cost", "Internal" and "T&M". I need to unpivot the all columns to the right of Column 1, 2 and 3, but include these additional categories by month.

For example there should be an additional column with the 4 variable fields above and should look something like this when unpivotted (the category column below is the new column).

The picture below shows the current shape of the data, any assistance you could offer would be hugely appreciated.
