Survey Response Data

I'm working with survey response data that looks similar to this:

Each question has the same set of responses (no freeform responses, just likert scale type responses).

In short, I'm wanting to provide a large table/matrix to summarize some of this data, with each service as a distinct row, the responses as the columns, and the customers as a slicer.  Something like this:

Any thoughts on how I can go about this while preserving the customer list for a slicer?

Two clarifying questions:

1) Do you want a DAX or M (Power Query) solution? In another words for BPI presentation or for data extracting level?

2) Can you send a small example set (your source data)?
