Newbie Needing Help w/ Modeling Data

Hello!  I am very new to Power BI.  A department wants some reports loaded.  The reports track patient visits per month on a calendar sheet someone created.  They also include a monthly visit goal.  I cannot for the life of me figure out how to organize the data from how she has the the table laid out.  I am going on week 4 of trying to figure it out without having to redo each monthly table for the past 2 years.  I have attached an example.  I can send the actual excel attachment but do not see where I can upload the attachment.  Please forgive me I am so new to Power BI 🙂 .  Thank you in advance.

@TheSweeper Not the nicest of data formats. What I would think would be that you are going to need to Transpose that source table. The difficulty is that I do not see anything in the source table that identifies the date or week. Plus, you might have to grab only specific rows in the query at a time and do this in multiple queries and then append them all together.
