Match value from a Matrix to create calculated column

Hi everyone,

I have a facts table as show below

I want to calculate column D with the help of below table

So if Severity score is 9 and occurance score is 3 then, then based on above ,matrix, output should be "Medium"

if Severity score is 7 and occurance score is 5 then, then based on above ,matrix, output should be "High"

and so on..

Is this possible to achieve using DAX?
