I'm trying to create a single line graph that plots each of these values but I'm struggling to figure out how.
Also how would I create a line graphy for say just 'CSAT Score'?

I'd suggest you need to bring the data in and then "Unpivot" the data within the "Edit Queries" section. This should leave you with description, date, and value columns.
Next you need to put the values into the "Values" area of the graph with the dates fields in the "Axis" area of the graph. Lastly, you can filter the visual to only display rows that contain "CSAT Score" in the description column.
