How to create a cross tab with Titles as shown below

Hello Community,

The below requirement may look easy ,but i am not able to figure it out. Kindly assist . 

I have 4 different queries consisting count and Percent values as shown in above screenshot . However, there are no titles or row headers as shown in first column ( Rule ) . How can i create those dummy names ( Partition ID , Is Exception , Serial Number and CI name ) and have those values pop up .

@Anonymous No idea from the information provided really. Maybe use an Enter Data query?

2. Expected output from sample data
