First date of each sales stage per opportunity and per Customer

Hi,

I am looking for the formula that brings the first date of each sales stage.

Hereunder you will find the database I have and the result I am looking for. 

What is the formula I need to use for the "Qualified Opportunity"? (first date of this company, this opportunit and sales stage = "Qualified Opportunity".
