Error displaying text in proper sequence in matrix

Hello, I am having problems dispalying text in the proper sequence. My expected result is to combine the 2 tables below in a matrix. 

However, its currently displaying the comments in a single line and the repeating it for the other 11 lines. Another one, is displaying wrong values but with proper comment sequence.

Is it the dax or relationship that is causing the problem? My pbix is attached below for reference. Thanks.

btw @camargos88 is there a way to show my actual values currently all the options display wrong values.

I do not see any relationship between them, that's the problem.

@camargos88 thanks solved it looks like I accidentally deleted the relationship.
