Error 500 when connecting to SharePoint hosted app as data source

Hey everyone,  I have a .pbix file stored in SharePoint Online.  The data source for the application is a SharePoint hosted addin whch has had some issues with refreshing, but nothing we couldn't find a workaround for.  These issues were mostly error 500s which we fixed by refreshing the dataset multiple times until it worked.  Sort of a brute force approach.

This has worked for some time, but I'm now experiencing an error where none of the lists in the SP hosted app work, and I'm wondering if anyone has seen something like this?

 

Does anyone have any idea what could be happening here?

Have you tried clicking the "Go To Error" button? The fix would depend on what step of the query it fails on. I'm guessing 1 or 2.

Source - did the URL change?

Navigation - did the file name change?
