Conditional Formatting - My formatting keeps disappearing

Hello SuperUsers:  So...for the past year or so my conditonal formatting on % calculations keeps disappearing- always the colors then the icons. They are based off a %.  My question is two fold: ( I am using May, 2021 but we will finally upgrade to July 2022 monday, the 11th of July.

 

1) can this be caused by a simple bug or as  a result of my calculation wich is simple division:

%10 Day Access = Calculate(Divide([10dayAccess], [patient_Completed_Appt], 0))
2) Can conditonal formatting be automated?

thanks for your reply; I addd conditional formatting based off the % Measure:

I want to show the % measure that has been met based off the % column which is a DAX measure

It works then disappears often them I must do it again which is a 2-step process as there is an ICON too.

Not clear what your expected output result ,could you share your sample data and expected output result.
