Is there a way for me to put an image (something like a question mark, for example) onto a live page and then have it open up a tooltip or another document or something that will allow me to show some definitions or documentation for that page?

In order to do this, you can add a hyperlink to a dashboard tile(including an image) as smoupre mentioned above. However, if you also need to do this in the report, I would suggest you to use a textbox instead and add a hyperlink to the textbox in the report like below.

You can do this in the Service. In the Service on a Dashboard, you can edit it using the ellipses (...) and create a custom link to any valid URL. Not really too many options in the Desktop so much along these lines although you can do something things with related tables. For example, create a table with a key and a text tooltip. Relate it to your table that your visual draws information from. Create a measure that uses something like IF(HASONEFILTER([key]),[tooltip],"") Display this measure in a table visualization or a custom visual like the "Long Text" visual. This way when they click on an element in the visual, your associated tip is displayed.
I tried to do that, however, I could not figure out how to get it to open a document on a server, which is ideal for what I want. Do you have an example of that?
I imagine that you could post a link to something like a document rendered in Word Online, Exel Online, etc. By using such a link, that would render the document on a server, or are you referring to opening a document on something like an internal file share? Probably not the case given the documentation here: https://github.com/Microsoft/PowerBI-visuals/blob/master/Tutorial/ExternalLibraries.md
So, second thought would be around some kind of security measure perhaps where the custom visual is being validated to run within the desktop client?
