I have two columns, as shown below:
I've currently calculated the "% Y" rate in the "RECOMMENDED" column using the following formula:

CALCULATE ( COUNT ( SA_Rec_Rate_Fact[RECOMMENDED] ), SA_Rec_Rate_Fact[RECOMMENDED] = "Y" ),
CALCULATE ( COUNT ( SA_Rec_Rate_Fact[RECOMMENDED] ), ALLSELECTED ( SA_Rec_Rate_Fact[RECOMMENDED] ) )
)
My question is this: How do I calculate the "% Y" rate for each indivdual month? I would like to visualize this as a line chart with the months on the X-axis (September 2019, October 2019, etc.) and the "% Y" rate on the Y-axis.

First, you can add one calculated column with formula:
in table, then applied this new added column in visual Axis and the measure
in visual Values just like in below screenshot:

Create a date table, Join you date with that and have month year there.
Create a month subtotal

To get the best of the time intelligence function. Make sure you have a date calendar and it has been marked as the date in model view. Also, join it with the date column of your fact/s. Refer :

Create a month column using MONTH([DATE_QUALIFIED]). Use that in your Axis and your measure in your Values.
