Adding transposed columns into new Table

Hi all,

 

I've set up an MS Forms that records responses from users in the below format:

I would like to transform this to look like the following output, updated whenever a new input is recorded in the MS Forms: (basically adds the latest row in a pivoted form to another table)

I've dabbled with unpivotting the dataset before without getting the wanted outcome, but this seemed to do the trick1
