This is so awesome!  But you forgot to post it also in the Power BI Birthday Gallery!

@Sandy - I tried that first. I've been trying, but the form doesn't seem to work on the Happy Birthday Gallery. I click the "Publish" button and it scrolls to the botttom of the page, grays out the button and doesn't do anything. Something is wrong with that form. I've tried it in Chrome, IE and Edge and it acts the same way.
