Hello,
I would like to first password protect the data and relationship tabs and then hide them so when the end user accesses the dashboard they can't change anything. I can't publish it because I work in healthcare and there is personal health information that can't go on the cloud. Any help with this would be greatly appreciated!

This is not a supported functionality right now. If you share reports with end users via sharing pbix file, any one can see and edit data model.
You could sumbit it as a feature quest here, if it is mentioned by many users, our product team will consider adding it to later release.

Is there a way to make pbix files read-only?

Unfortunately this isn't a feature. Power BI Desktop is more for power users, not necessarily report consumers. I believe the Office 365 Suite is entirely HIPAA compliant, but I understand the concern for security.
Realistically, Power BI Premium w/ Report Server would be a good fit because it would allow you to run the full PowerBI.com environment on your local hardware, removing the concern over cloud.
